Stay information

Documents Required for Inpatient Registration:

  1. IC Card or Insurance Card
  2. Personal Identification Card
  3. Personal Stamp (when available)
  4. Hospital Admission Consent Form*

      *   Before registering for hospital admittance, please sign a Hospital Admission Consent form.  Hospital Admission can be found at the following counters:
  

         Shalu Branch: Hospital Admission is at counter number 17 on the 1st floor.
   Dajia Branch: Hospital Admission is at counter number 12 on the 1st floor.

 

Financial Assistance
According to the Bureau of National Health Insurance, if you have any of the following, 
you may apply for financial assistance:

  • Serious illness (must provide Certificate of Diagnosis)
  • Child delivery (handbook for maternity information)
  • Workplace disability (must provide proof from employer)

Patient Responsibilities
 
1.  If you need to leave the hospital for any reason, you must follow the National Health 
         Bureau’s regulations for requesting a leave:

  • A doctor must sign a 「Permission to Leave the Hospital   Agreement」and you must 
    complete all paperwork before leaving  
  • When you return, you must report to the Nurses’ Station to check in
  • You cannot leave the hospital more than two times during your hospital stay, for a maximum of four hours each time
  • You cannot leave between the hours of 10:00 pm to 6:00 am
  • If you do not return at the afore-mentioned time or if you do not follow the procedure for
    requesting a leave, the hospital is not responsible for any events that occur during this time 

2. In order to preserve the peaceful atmosphere of the hospital, be considerate of other 
    patients’rights to a quiet environment

3. In order to allow the patient to rest, visitors must leave by 10:00 pm.  Only one family 
    member may stay overnight to accompany the patient.  The overnight visitor must register 
    at the Nurses Station staff and request a 「hospital Entrance/Exit Pass」from a nurse.
4. For your safety, please do not bring valuable possessions or excessive amounts of cash.
    If you notice an unfamiliar person in your hospital room, please alert the nursing 
    staff immediately. 
5. For everyone’s safety, do not bring electronic items for personal use or cook in your 
    hospital room. 
6. Do not chew beetle nut, bring animals, or smoke in the hospital.  Patients and family 
    members must follow these rules in order to preserve the healthy environment of the  
    hospital. 
7. Please refrain from using cell phones, as they will interfere with our electronic 
    medical equipment. 
8. If you require a wheelchair, please rent one from the Nurses Station. 
9. Please do not give the hospital staff any gifts or money. 
10. If you have any comments or questions about the hospital, please go directly to the
      Nursing Staff; or fill out a Patient Comment Form.  
11. If family members would like an update on the patient’s condition, go directly to the
      Nursing Staff who will arrange a meeting with the doctor; or locate a doctor on duty,
      who can discuss it with you.    


Provisions
    1.  The hospital provides a dining service.  You can order hospital-prepared meals from the Nursing Staff (pay by the meal) 
    2.  There are convenience stores nearby both Dajia and Shalu Branches, a restaurant and
          bakery in the Dajia Branch, and a coffee shop near the Shalu Branch
    3.  Patients should bring their own loose clothing 
    4.  Essential toiletries for patients in Single and Double Occupancy rooms will be
          provided free of charge.  Those patients staying in National Health Insurance standard beds should bring their own toiletries
     5.  There is a water fountain in front of every Nurse’s Station
     6.  We have laundry machines for patients or their families to use during hospitalization
          (Shalu: 2nd, 5th, 6th, 7th, and 9th floors.  Dajia: 8th and 10th floors)
     7.  There is hot water available for showering 24-hours a day
     8.  For any personal items you may require at the Shalu Branch, there is a twenty-four
          hour convenience store and a medical supplies store on the first floor. At the
          Dajia Branch there is a twenty-four hour convenience store and medical supplies
          store on B1.
     9.  Ice machines are also available on many floors.
         (Shalu: 2nd, 5th, 6th, 7th, and 8th floors. 
          Dajia: 7th and 9th floors.)

Safety and Hygiene Precautions
     1.  Entering and Exiting the Hospital After Hours: 
          In order to provide a safe environment for everyone, visitors that need to enter and
          exit the hospital after hours (10:00 pm to 6:00 am) should use the
          Emergency Room doors and bring their Entry/Exit Pass to be checked by security officers
     2.  Unauthorized vendors are restricted from entering the hospital.If you see any vendors
          selling medical items, posting advertisements on the walls, or anyone who compromises your safety within the hospital environment, please inform a nurse or call a
          security guard (Shalu: extension 2193 or extension 9; Dajia: extension 2195 or extension 9)
     3.  Special Rules for Overnight Visitors:
          In order to preserve the patient’s safety and avoid accidents, all patients should have a friend or family member to accompany them

In respect for other patients, all patients and visitors should turn off all televisions and radios after 10:00 pm.
    4.  All hospital rooms contain the following items for your use:
     Call Button—Press the red Call Button at the head of the bed in order to speak with someone fromthe Nursing Staff.
           •   Emergency HandleWhen in the washroom, pull the emergency strap on the wall
                near the toilet.  Someone from the Nursing Staff will be there shortly.
