Procedure for Hospital Discharge

In accordance with National Health Insurance Regulations, if a doctor recommends that you be discharged, please comply.  All fees and charges incurred by patients who choose to remain in the hospital at this time will not be covered by health insurance. In order to protect your rights as a patient, please follow the discharge and transfer procedures described to you by the Nursing Staff.

Steps

Procedure Details

1

Your physician will determine when you are well enough to leave the hospital and will write the discharge order.

If you are requesting to be discharged without the doctors approval, you must sign an Against Advice Discharge (ADD) Consent Form.

Your nurse will inform you when the discharge process is complete and inform you of any special instructions.

The doctor will write a Certificate of Diagnosis. Please see #7 for instructions to getting an Official Certificate of Diagnosis, or making duplicate official copies.

You will be given a bill, pharmacy order, and an Outpatient Clinic Appointment Card if the doctor has decided that you need a follow-up visit.

Daytime Discharge: If you are discharged before 9:30pm, please go to the Inpatient Cashier on the 1st floor (Sha Lu Branch: Counters # 6, 7; Da Jia Branch: Counters #13, 15). The cashier will ask you for your Health Insurance Card; please tell her if you do not have one. Once you have settled your account, you will receive a receipt.

Weekend/Evening Discharge: If you are discharged after 9:30pm on a weekday, or on the weekend, please go to the Emergency Room Registration Counter on the 1st Floor.

For an Official Certificate of Diagnosis, you must take the Certificate of Diagnosis the doctor has written and a form a personal identification (Passport or ARC) to the Certificate of Diagnosis Counter (Sha Lu Branch: Counter #5; Da Jia Branch: Counters #13, 15). For additional copies of your Official Certificate of Diagnosis, you may ask the representative to make duplicate copies for you with an additional fee.

Take your receipt from the Cashier which will have your waiting number for the Pharmacy. You may pick up your medications when your number appears on the screen. Please follow the instructions printed on the package and be sure to ask the pharmacist if you have any questions.

Once you have received your medication, you may leave the hospital.
