Qualified Applicants:

1.    Those under home (self) isolation/quarantine who need to go out for reasons, such as visiting relatives in a critical condition, attending a relative’s funeral, or dother urgent issues.

2.    Those who need to travel to other countries for the reasons listed above.

3.    Job requirements

4.    Short-term business travelers

5.    Students studying abroad

6.    Foreign nationals, Mainland Chinese, or Hong Kong/Macao residents who are departing from Taiwan

7.    Family members of people traveling abroad for the reasons listed above

8.    Those approved by the Central Epidemic Command Center

9.    Other issues:__________

FOR ALL THE THOSE ABOVE, YOU MUST NOT HAVE SYMPTOMS OF COVID-19

Self-pay COVID-19 Test Fee (Standard): $4,000 NTD

Documents Needed (depending on your reason):

1.    Two IDs (ARC, Passport, National Health Insurance Card…etc)

2.    Self-pay COVID-19 test form

3.    E-Flight ticket or proof of tickets purchased

4.    Work Certificate/Visa

5.    Student Visa/ Student ID/Enrolment Certificate

6.    Proof that your COVID test is required by the Taiwan CDC

Procedures:

1.    Bring your IDs and required documents to Kuang Tien General Hospital’s Emergency Room (ER) and register at the counter.

2.    Make payment(cash or Taiwan's credit card only) at the cashier (Standrad:NTD$5000 ,Priority:NTD$6000 )

3.    The physician will administer the test

4.    Standard:Go back to the ER 48 hours later(tomorrow, after 21:00 pm ) and bring your ID.

      Priority:Go back to the ER at the same day (after 21:00 pm ) and bring your ID

Dajia Branch:Every Day, 24 hours. 

Dajia Branch: Moday to  Friday from 08:00-14:00am.Saturday and Sunday from 08:00-11:30am.

Notes:

1.    You must be a qualified applicant for the test and have the required documents prepared.

2.    You can apply for a test once every three months.

3.    When you come back to Taiwan, you must follow the epidemic prevention (home isolation / quarantine) guidelines by the Taiwan CDC.

4.    COVID-19 test/PCR is a self-pay service. If you have other medical requests, please visit our ER or outpatient clinics.
