| The 58 year-old male patient complained about intermittent nasal bleeding and loss of smell for about 3 months. He visited our OPD of ENT. Dep. with series of examinations done. Endoscopic biopsy showed possible malignant tumor. Then, CT and MRI of brain were performed, showing one big mass over olfactory bulb with brain and nasal invasion. He was referred to N.S Dep. for further evaluation. After delicate preoperative planning and discussion with ENT. doctor, combined surgery was adopted by bicoronal craniotomy with removal of the olfactory tumor by me and endoscopic removal of intranasal tumor by Dr. Lai. This ends up with a successful and satisfactory result. Total removal of the tumor without any complication was achieved. Pathological examinations proved the tumor to be an olfactory neuroblastoma. Post-operative adjuvant chemotherapy and radiation will be beneficiary statistically. Hence, we bring up the case and have a further discuss about the operative approaches, the tumor and treatments. |
