Medical Record and Imaging Application

1. Applicant: the patient himself, the legal representative of the minor, and the legal heir of the deceased.
2. Required documents:
2.1 Application in person: ID card (original).
2.2 The minor’s information is applied for by the legal representative: (1) Original of the legal representative’s ID card (2) Document proving the relationship between the legal representative and the patient (household register or original of the patient’s ID card).
2.3 The deceased’s information must be applied for by the specific heir: (1) The original ID card of the legal heir (2) Documents proving the relationship with the patient (3) The patient’s deregistration certificate (household registration transcript or death certificate).
Note: To apply as an agent on behalf of the entrusted party: the entrusted party must hold the original ID card of the applicant, a written consent document signed by the applicant in person, and the original ID card of the entrusted party. "Patient Entrustment Consent Form" (please download the consent form and sign it. You will need to attach it when you get the information).
3. The content of the medical records applied for will only be retained for 30 days from the date of application. Fees cannot be refunded if the application is not received within the expiration date. If necessary, please reapply and be billed again.

4. Applicants who are hospitalized: Please prepare the second item of information mentioned above to apply to each nursing station.
